Payer note elements
List the medical necessity elements each payer expects on every visit note, so signing holds or warns when one is missing.
Some payers expect every visit note to show the same things, such as the session length, the codes with their units and the goals worked on. Setup > Payer Note Elements keeps one list per payer, and NeatPractice checks it each time a visit note billed to that payer is signed. Changing the lists needs the Setup: Clinical Manage permission.
Adding a list#
- Click New List.
- Pick the Payer. A payer has one list; edit it to change what it asks for.
- Choose the Required elements:
| Element | Met when |
|---|---|
| Start and end times | The visit has its start and end times |
| Session length | The visit lasted more than zero minutes |
| Codes with units | The visit has codes, each with units |
| Place of service | The visit records its place of service |
| Diagnosis | The visit carries at least one diagnosis |
| Rendering provider credential | The note's provider has a credential on file |
| Goals addressed with a measurement | At least one goal is marked addressed with a value or counted trials |
| Session narrative | The note's session narrative has text |
| People present | The visit lists the people present |
- Leave Disciplines empty to check every discipline, or pick the ones the list applies to.
- Turn on Hold signing until met to stop signing until every element is there. Turned off, signing shows a warning for each missing element and goes ahead.
- Click Create List.
To note where the list comes from, such as a section of the payer's manual, turn on Names its source.
When it is checked#
The list applies to notes written on a visit whose payer it names. Missing elements appear under Before signing in the note, each starting with the payer's name, and in the sign dialog. Notes without a visit or a payer are not checked. A change to a list applies the next time a note is signed; signed notes stay as they are.
A payer's list adds to the elements your clinic requires on every daily note (see Note elements), where these lists also show. An element on both lists is checked once, and it holds signing because your clinic requires it.
Applied behavior analysis (ABA) session notes are not checked against these lists. They use their own medical necessity elements, set per payer in Note elements.
Turn a list off with Deactivate in its row menu to stop checking it without losing it.
