Discipline module settings
Set how long a plan of care runs, when recertification and progress reports come due, whether evaluations propose a re-evaluation date, and when the Medicare therapy threshold alert shows, for each discipline module such as speech, occupational therapy, physical therapy and mental health.
Each discipline module your clinic has, such as speech-language pathology (SLP), occupational therapy (OT), physical therapy (PT) and mental health, has its own settings page. The pages start from the Medicare rules, so a clinic that bills Medicare can leave them as they are. Change a value when your clinic or your state follows a different rule.
Open Setup > Modules and pick the discipline. A page shows only while your clinic has that module. Anyone with access to Setup can see the settings; changing them needs permission to manage clinical setup. Click Save Changes at the bottom of the page when you are done. The help next to each field shows the value the module starts from.
Applied behavior analysis has its own page with different settings, including how long a new treatment plan runs: see ABA settings.
Plan of care#
- Certification length (days): how long a new plan of care runs from its start date (90 days to start). When the patient's primary coverage on the start date is Medicare, a new plan still proposes at most 90 days.
- Recertification due (days before the end): how many days before the plan ends the recertification comes due (10 days to start). The Recertify by date on the plan, the provider's due items on the schedule and Recertifications due in Oversight follow it.
See Plans of care.
Progress reports#
A progress report is due every so many treatment visits or every so many days, whichever comes first.
- Every (treatment visits): 10 to start. Leave it blank to count only days.
- Every (days): 30 to start. Leave it blank to count only visits.
One of the two must be set. Both are warnings: they apply to payers other than Medicare whose billing rules use the discipline default, and to patients whose payer has no billing rules. A payer that sets its own visits or days uses those instead.
The Medicare rule does not change here. Under a payer with the Medicare rule the daily note shows Progress report due from treatment day 8, and on treatment day 11 it cannot be signed until a therapist signs a progress report. See Progress reports.
Re-evaluation#
Turn on Proposes a re-evaluation date and enter Re-evaluate every (days) to have an evaluation or re-evaluation signed without a re-evaluation date get one that many days after its date. The re-evaluation then shows in the provider's due items and in Re-evaluations due in Oversight. A date the therapist enters on the evaluation always wins. It is off to start.
Medicare therapy threshold#
This panel shows for disciplines that count toward the Medicare therapy threshold (speech, occupational and physical therapy). Medicare requires KX on claims past the threshold, so it cannot be turned off for a discipline: every payer whose billing rules track the threshold adds KX once a patient crosses it.
- Alert at (percent of the threshold): 80 to start. The Therapy Threshold dialog shows Past 80% (or your number) once a patient's yearly total reaches that share and has not crossed the threshold yet. When two disciplines share a total, the lower number applies.
Threshold for the current year shows the amount Medicare set for the year. It cannot be changed here.
Shared settings#
The rest of a discipline's rules live on the Setup pages every discipline shares. The Shared settings panel links to them: Documentation rules and the Goal library open on this discipline; Templates, Therapy activities, Outcome instruments, Supervision, Credential types and Intake list every discipline together.
