Addenda and late entries
Correct or add to a signed document with an addendum, fix a visit's billing details, and see which documents were signed late.
A signed document never changes. To correct it, add detail, or record something that was not written at the time, you add an addendum. The original stays exactly as it was signed, and every addendum appears after it, in order, on the screen and on the signed copy.
Add an addendum#
- Open the signed document from the patient's Documentation tab.
- Click Add Addendum at the bottom of the document.
- Choose the Kind:
- Correction fixes something the document said.
- Clarification adds detail to what it said.
- Late entry records something that was not written at the time.
- Enter the Reason and write the Addendum.
- Click Continue, read the attestation, check I agree to this attestation and, when asked, enter your verification code.
- Click Sign.
If you click Cancel on the signing step, the addendum window opens again with what you wrote and any billing correction, so you can change it and continue. It is kept until you sign it or leave the document.
The addendum is signed the same way a document is: your name and credentials, the server time, how you confirmed it was you, and a fingerprint of the addendum's content. The signed copy is saved again with the addendum in it, so prints, secure emails and faxes of the document include every addendum.
You can add an addendum to a document you are the provider on, or one you supervise. Users who can addend every document in the clinic can add one to any signed document.
Correct a visit's billing details#
When the document is a visit's note, the addendum can also correct what the visit bills:
- In the addendum, turn on Correct the billing details.
- Change the Place of service, the visit's diagnoses or the treatment lines (codes, minutes, units, modifiers and diagnosis pointers).
- Continue and sign as above.
NeatPractice recalculates the units, matches each line to its authorization again and tells billing what changed, so billing can update a charge that has not been billed or review a corrected claim for one that has. A finalized visit shows Addended once it has an addendum.
Co-signing addenda#
When the original note was already co-signed by a supervising provider, the author's addendum waits for that supervisor too. Until the supervisor co-signs it, a billing correction is not applied and the visit's charges are held.
To co-sign an addendum, open the note from Awaiting my co-sign on the Todos page, click Co-sign Addendum, check the attestation and click Co-sign.
When a note is still waiting for its own co-signature (for example after the supervisor returned it), the addendum needs no separate co-signature: the supervisor's co-signature on the note covers it.
Late entries#
A document signed more than seven days after its date is marked Late entry, on the document and in the patient's Documentation tab. The count uses the clinic's time zone. A Late entry addendum is its own kind of addendum and says so on the document.
