Caregiver feedback

Create monthly caregiver acknowledgment and feedback forms for many learners at once, share their links, track answers and complete them in person.

Each month, caregivers confirm they reviewed their child's progress and rate the services they received. NeatPractice creates one form per learner for the month. A caregiver answers it from a link, or in person on a staff device.

The link names your clinic and the month only, never the learner, so it is safe to text or email.

Create the month's forms#

  1. Go to Documentation, then Caregiver feedback.
  2. Click Create Forms.
  3. Choose Every active learner (learners with an open ABA episode at the location selected in the header) or One learner.
  4. Pick any day of the Month the feedback covers and how many days the link works.
  5. Click Create Forms.

Learners who already have a form for that month are skipped, so you can run it again safely after new learners start. Each form is meant for the learner's primary caregiver, else their legal guardian, as listed on the chart's contacts.

Open a row's actions and click Copy Link, then send it to the caregiver. The caregiver rates five statements from 1 (strongly disagree) to 5 (strongly agree), can add comments, confirms they reviewed the month's progress and types their name to sign. A link works once, until it expires.

Track answers#

The Status column shows where each form stands:

StatusMeaning
Not openedThe link was not opened yet
OpenedThe caregiver opened the link but has not sent answers
CompletedThe caregiver answered by link or in person
ExpiredThe link stopped working before anyone answered
RevokedSomeone at the clinic closed the link

Click View Answers on a completed form to see the ratings, comments and who signed. Average rating helps you spot a family that may need a conversation.

Read the feedback at a glance#

Cards above the list sum up completed forms: how many, the average rating, the tone of the comments and how many need follow-up, with the themes caregivers mention most. In the list, the Feedback column marks each completed form:

  • Positive, Neutral or Negative, from the ratings and the words in the comments.
  • Themes such as communication, scheduling, staff, progress, caregiver training, billing or safety, from the comments and from statements rated 1 or 2.
  • Needs follow-up when a comment mentions harm, safety, leaving services or a complaint. Call the family soon.

These marks come from fixed word lists, not from reading the comment the way a person would, so always read the answers before acting.

AI insights for a learner#

For a reading of one learner's feedback over the months, open the learner's chart, then Contacts > Caregiver guidance. The Caregiver feedback insights card reads the completed forms (up to the last twelve) and says how caregivers feel and whether that changed, what needs follow-up and the themes that recur.

  • Nothing is sent until you click Generate; Refresh writes it again after new answers arrive. It uses assistant credits.
  • It is marked Written by AI and is informational only: it never changes a plan, holds nothing and suggests no treatment. Each line links the form it came from, which opens with its answers on this page.
  • Your clinic turns it on under Caregiver feedback insights in Documentation rules. A learner opted out of AI, or one without AI consent when your clinic requires it, gets no insights.

Complete a form in person#

When the caregiver is at the clinic, click Complete in Person, hand them the device and let them answer and sign. You are recorded as the witness. A form whose link expired can still be completed in person.

If a link went to the wrong person, click Revoke Link. The link stops working at once, and you can create a new form for that learner and month.
