Deletion Reasons

Keep the list of reasons staff pick when they delete a record, and see each delete's reason in the change history.

When someone deletes a record in NeatPractice, such as a contact, a document or a list entry, the confirmation can ask why. Open Setup > Deletion Reasons to keep the reasons offered.

A new clinic starts with Entered in error, Duplicate, Wrong patient or record, No longer needed and Other.

Add or change a reason#

Click New Reason, enter the Reason and an Order (lower numbers come first), and save. Turn off Active to stop offering a reason; deletes that already used it keep naming it.

Require a reason#

By default the reason is optional. To make every delete name one, turn on Ask for a reason on every delete in Practice. The Delete button then stays off until a reason is picked.

The rule holds everywhere, not only in delete confirmations. A delete that starts from a small trash icon, a bulk action or a connected tool is refused until it names a reason, and NeatPractice asks you for one before finishing it. If the reason you picked was turned off in the meantime, NeatPractice asks you to choose another. Removing a logo or an image, clearing a saved view or signing out a session is not a record delete and never asks.

Where the reason shows#

NeatPractice never truly removes clinical or financial records; a delete hides the record and keeps its history. The Audit changes report lists each delete with the reason it was given, so an auditor can see who deleted what, when and why. Set its Action to Deleted to see only the deletion history, and filter by Record type or User to narrow it.

Managing the list needs the Setup: Practice Manage permission.
