Service addresses

Record the places outside the clinic where a patient is seen, such as home, school or daycare, with the place of service claims report.

The Service addresses tab on a patient's chart lists the places outside the clinic where the patient is seen. When a session happens at one of them, claims report that address and its place of service code.

Add a service address#

  1. Open the patient's chart and click the Service addresses tab.
  2. Click Add Address.
  3. Choose a Label: Home, School, Daycare or Other.
  4. Add a Name if it helps, such as the school's name.
  5. Enter the street address, city, state and the full nine-digit ZIP+4.
  6. Leave Place of service blank to use the usual code for the label, or pick another code.
  7. To measure EVV clock distances from this address, turn on Map coordinates and enter its latitude and longitude (copy them from any map app), or click Verify Address to fill the ZIP+4 and coordinates for you (Verify an address). Without them, EVV shows the address as not geocoded.
  8. Click Add Address.

Change or retire an address#

Open the row's menu and click Edit. When the patient is no longer seen at an address, turn off Active instead of deleting it, so past sessions keep the address they were held at. Use Delete only for an entry made in error.

Anyone who can edit the chart can change service addresses.
