Patients
Progress
Follow a patient's progress on the chart, give your clinic's outcome instruments, see each result and its band, and follow the scores over time.
The Progress section of the chart shows how a patient is doing over time. Open a patient's chart and choose Clinical > Progress. You need access to the patient's notes to see it.
Its Outcome measures list holds every result of the outcome instruments your clinic defines and draws a trend for each instrument. When your clinic offers applied behavior analysis (ABA), a Graphs tab beside the outcome measures holds the learner's program graphs.
Give an instrument#
- Click Give Instrument.
- Choose the Instrument, the Date given and who Answered by: the patient, a parent or caregiver, a teacher, a clinician or someone else. Click Add a note for anything worth keeping with the result.
- Answer the items. The charted score and its band update as you go.
- Click Save Result.
NeatPractice scores the answers with the instrument version shown and saves the scores and bands with the result. A result is never scored again, even after the instrument changes.
Send a rating form by link#
A parent, teacher or someone else can answer an instrument from home or school, without an account:
- Under Rating form links, click Send Rating Form.
- Choose the Instrument, who it is Answered by and the Respondent's name. Open for (days) sets how long the link works (14 days unless you change it). Click Add a message to write a note shown above the form.
- Click Create Link. The link and a six-digit access code show once.
- Click Send Link, enter the respondent's email address and click Send Link again. The email holds only the link and a short note from your practice: no patient name, no message and no access code. Or click the copy button and send the link yourself; it names no patient.
- Give the access code separately, by phone or in person. The form does not open without it.
The respondent opens the link, enters the access code and answers the items; the form shows the patient's first name and last initial only. Their answers become a result dated the day they send it, scored like one you record, and the Outcome results list shows who answered, marked by link. Five wrong codes lock the link.
The list shows each link's status: Sent, Opened, Completed, Expired, Revoked or Locked, and under Emailed the address the app last sent it to, when and by whom. To email an open link (again, or to a new address), choose Send link in its row menu. To close an open or locked link, choose Revoke in its row menu. Scores-only instruments have no items, so they cannot be sent.
Trends#
Trends draws one chart per instrument and person who answered, so a parent's and a teacher's ratings are never on the same line. Each chart follows the instrument's charted score by date, the same way a goal's progress chart does. Point at a dot to see its date, score, band and version. The card shows the first and latest result and how many there are, and notes when results were scored with more than one version. Click Enlarge at the top right of a chart to see it over the whole screen, or Export to save it as a PDF or PNG image with every result listed under it.
View or void a result#
Click View on a result to see its answers, every score and band, and its note. To void a result recorded in error, open the More actions menu (the three dots) and choose Void Result, then give a reason; the result leaves the list and the charts. A result recorded in a signed document is part of that document and cannot be voided.
In notes, evaluations and progress reports#
A document whose template has an Outcome measure block lists the results given in that document. While the document is a draft, click Give Instrument in the block to record one; the date starts at the document date. Giving or voiding a result there follows the same rules as editing the draft: its author, someone who can edit any draft, or someone who took it over. Everyone else sees the results read-only, and results in a draft are voided from the draft, not from the chart page. Signing the document locks its results into the signed copy.
A signed or printed progress report or discharge summary also lists every result of the patient in the report's period, with a chart per instrument and person who answered. Results given later do not change a signed report.
