Setup

Online intake

Publish a public request form on your website so new families can ask for services, and review each request before a chart exists.

An online intake form lets a family ask for services from your website without signing in. Open Setup > Online Intake to make one, then put its link on your site or send it to referral sources.

Making a form#

Click New Online Form and fill in:

  • Name: what staff see in lists, such as New patient request.
  • Patient form: the template of kind patient form whose questions the family answers. Fields bound to the chart (name, date of birth, guardian, insurance) fill the new chart when staff accept the request.
  • Services offered: the disciplines the family can choose from. Choosing ABA also asks for weekly hours and settings.
  • Instructions (optional): shown above the form.
  • Confirmation message (optional): shown after the family sends the form. A standard thank-you appears otherwise.
  • Active: an inactive form stops taking requests, and its link says the form is not available.

The list shows each form's link with a copy button and how many requests are waiting. Managing forms needs the Setup: Practice Manage permission.

What families see#

The page names your practice only. The family answers the questions, attaches files where the form asks for them, picks the service and the days and times that work, types their name and relationship, and confirms the answers are true. Nothing becomes a chart yet: each request waits in online requests for staff.
