Documentation
Documentation across patients
See the clinical documents of every patient you can see on one page, narrow them by patient, type, status, discipline, provider or date, and open or start a document without opening each chart.
Open Documentation in the Docs panel of the main menu to see the clinical documents of every patient you can see, newest first, at the location you are working in. It is the same list as the Documentation tab on a patient's chart, with the patient added, so you can move from one patient to the next without opening each chart.
The documentation waiting on you, such as your unsigned notes and the notes to co-sign, is on the Todos page instead (see Sign and co-sign notes).
Read the list#
Each row shows:
- the document date; click it to open the document
- the patient and chart number; click the name to open the patient's Documentation tab
- the type (such as evaluation or daily note) and the episode of care
- the discipline, the provider and the template
- the status, with the co-signature state, the number of addenda and a Late entry mark where they apply
- when the document was last saved
Click the date to open the document in the same window you use on the chart. Open the row menu to Open the document or, for an evaluation, re-evaluation or plan of care, to Copy Into New Draft.
Narrow the list#
- Pick patients in the patient panel to see only their documents.
- Pick providers on the panel's Providers tab to see only their documents. The tab shows to users who can see the provider list; everyone else filters by Provider under Filter.
- Pick a type in the Type menu beside the search box to see only that kind of document; All types shows them all again. It is the same filter as Type under Filter, so changing one changes the other.
- Check one or more disciplines in the Discipline menu to see only their documents. It is the same filter as Discipline under Filter.
- Click Filter to narrow the list by Date, Type, Discipline, Provider or Status.
- Click a column header to sort by it.
- Type in the search box to find documents by a patient's name or chart number, a provider's name, a template name or
a document date, such as
2026-09-15or09/15/2026.
Start a document#
Pick exactly one patient in the patient panel, then click New Document. The steps are the same as on the chart (see Start a document), and the new draft opens right away. With no patient or several patients picked, New Document stays unavailable.
Open Plans of care in the Docs panel to see the plans of care across your patients.
Export charts#
To hand several charts to an auditor or another provider, pick the patients in the patient panel (up to 100), open the page's menu and choose Export Charts:
- Document types: leave empty for every type, or choose the ones to include.
- From and Through: the dates of the documents.
- School contract records (with the school records permission only): documents of episodes served under a school contract are left out. Choose to include them, or to export only them.
- Protect the ZIP with a password (optional): enter a password of 8 to 128 characters twice to encrypt the ZIP with AES-256. NeatPractice does not keep the password, so if it is lost, run the export again. Share it separately from the file. My exports marks the file Password protected.
Click Start Export. NeatPractice builds a ZIP file in the background with one PDF of signed documents per chart, named by chart number, and a list of how many documents each holds. You get a notice when it is ready; download it from My exports in Reports within 24 hours. Only signed documents you may open are included, and each chart in the file is recorded in the access log. Exporting needs the Patients: Chart Export permission.
