Note oversight reports

Find signed notes that repeat another note by the same provider, and follow notes returned to their authors for correction, with CSV export.

Two reports in Reports > Clinical and compliance help supervisors and compliance staff check documentation across the clinic, for every discipline. Both need the Review permission for documentation audits as well as the clinical reports view. With caseload access, you see your own caseload only (a supervisor's caseload includes their supervisees'). The Clinical Supervisor and Behavior Analyst roles have both for their caseload.

Run them like any report: pick the filters, click Run report, and export a CSV, Excel or PDF file. The exported file has the same columns as the screen.

Note similarity#

Payers and auditors treat notes that repeat one another word for word as a sign of copied documentation. This report finds signed notes whose text closely matches another signed note by the same provider.

  1. Choose the Note dates (up to 92 days) and how alike the notes must be in Similar at least (70%, 80%, 90% or 95%; 80% by default).
  2. Narrow by Providers, Disciplines or Locations if you like, and click Run report.

Each row is a pair of notes of the same type (two daily notes, two evaluations) by one provider, for the same patient or two different patients, both dated in the range. The newer note comes first, then the Similarity (%) and the older Matching note. Click the note date to open the newer note, or the matching note date to open the older one. The closest pairs are listed first.

NeatPractice compares the text answers of the notes as they were signed, the same way the similarity check does before signing. Very short notes are left out, since a few words of shared wording would match almost anything. The report shows names, dates and the percent only, never the note text.

Returned notes#

This report lists every time a note was returned to its author for correction in the date range, whether a reviewer returned it from Note reviews or a supervisor returned it instead of co-signing.
