Activity flowsheets
Record the exercises and drills performed in a session with their dose and support, carry them forward and follow them across sessions.
An activity flowsheet lists the activities you performed in a session, each with what it records: sets and repetitions, minutes, equipment, trials, assistance or cueing, plus the patient's response. The activities come from your clinic's therapy activity library.
A note has a flowsheet when its template includes an Activities block. The starter speech and occupational therapy daily notes include one under Activity flowsheet; to add it to your own templates, see Templates.
Record activities on a note#
- In the flowsheet section, click Add Activity and pick an activity. Its defaults fill in.
- Enter what you did: for example Sets and Reps, or Correct out of Trials.
- Optionally pick the Goal from the plan of care the activity worked toward, and type the patient's Response.
Drag rows to reorder them, or click the trash icon to remove one. The flowsheet saves with the rest of the draft.
- Trials read as correct out of attempted, such as "8/10 trials (80%)". With no attempts the activity has no accuracy, never 0%.
- Correct trials cannot be more than the trials attempted; the field shows what is wrong and the rest of the note keeps saving.
See the last session and copy it forward#
Under each activity, Last session shows what the patient's latest earlier note of the same discipline recorded for it, dose and response, so you can set today's dose against it. Only notes you may read count: when your role sees only your own notes, it is your latest earlier note (see Other providers' visits and notes).
Click Copy Last Session to bring in every activity from that note that is not on today's flowsheet yet, with its dose, support, equipment and goal. Response stays empty: each note must describe how the patient did on that date, so write today's response yourself (the last one stays visible under Last session). Copied rows carry a Copied from label with the date of the note they came from. The label goes away as soon as you change the row, so a reviewer can tell which entries were carried over unchanged. Activities your clinic has since deactivated are not copied.
After signing#
Signing freezes the flowsheet with the rest of the note, and the signed PDF lists each activity with its dose, support and response. To correct an activity after signing, add an addendum.
Follow activities across sessions#
Open the patient's chart and choose Flowsheets to see every activity recorded on the patient's notes, one row per activity and one column per session for the latest 12 sessions, oldest first. Each cell shows what that session recorded. Drafts are private work, so the grid shows signed notes plus your own drafts, which carry a Draft badge; another provider's session appears once they sign it. When the patient has more than one episode of care, use Episode to show one.
