Payer reports
Write evaluation, progress and reauthorization, and discharge reports for the payer in any discipline, take the caregiver's signature, sign, submit and record the payer's decision, or record a report written elsewhere.
The chart's Reports tab lists the reports a payer reviews, for every discipline: evaluation or assessment reports, progress and reauthorization reports, and discharge summaries. Each report is a clinical document of the matching kind plus the steps the payer sees. A discipline can add its own report content; ABA's is described in ABA reports.
Start a report#
- On the patient's chart, open Reports and click New Report.
- Choose the report, the episode and the report date.
- For a progress report, choose the treatment Authorization it asks to renew.
The report period defaults to the authorization's dates. Turn on Set the report period to choose other dates. The period appears in the document's reporting period section when its template has one. The document opens so you can write it on your clinic's template for that kind and discipline, or on the payer's own layout when one of your forms is set up for that payer (see Payer layouts).
Caregiver review and signing#
When the payer's rule set has Caregiver signs reports turned on (see Payers), a new report needs the caregiver's signature before the provider signs. You can also turn it on or off per report in Edit Details, where you also change the period and the authorization of a draft.
- Click Send to Caregiver on the draft.
- After the caregiver has read it, click Caregiver Signs and hand them the device to type their full name and relationship.
- Sign the document as usual. The report becomes Ready to submit.
Submit and record the payer's decision#
- Record Submission: the date, how it was sent (payer portal, fax, secure email, mail or other) and any reference number. The report becomes Submitted.
- Record Decision: Approved or Denied, the date and a note.
A signed report changes only by addendum.
Reports written elsewhere#
To record a report written and signed outside NeatPractice, first add the PDF to the patient's chart files, then start a new report, turn on Written and signed elsewhere and choose the file. It is ready to submit at once and has no document to write here.
Reauthorization reports#
A progress report that names a treatment authorization is the reauthorization report for it. The Reauthorization reports due dashboard widget shows In progress once one is started, and the item clears when the report is signed.
Permissions#
Viewing reports needs Clinical note: Read; writing them and recording each step needs Clinical note: Author. Another provider's report follows your role's Other providers' visits and notes setting: with Own only it is not listed, and with List only it is listed with a lock and cannot be opened or changed.
